Drone crashes - internal investigations

I was wondering if any of the LE using drones have a template for pilots to fill out if they crash a drone? This form would be for them to annotate the weather, time, location, altitude, reason for flight.....things like this. This would be used for internal investigations for our department and not the FAA. Thanks.

Obviously the pertinent details are date, location, weather, consequences, pilot's statement and signature. Here's a resource if you want to build a custom form.

Rather than a form, our pilots are required to send a mission recap/pilot report email after every mission to a select group of managers. The emails become part of their individual pilot record as well as for the organization. The emails must contain all of that information you mentioned and more.

If there is a crash event or another anomaly of any kind, then that information is included as well, in extreme detail and becomes the basis for any investigations.

I like it! Though our pilots are required to fill out the mission up to completion before they can ever take off. The software is set for required fields and the mission is not available until they do so. Extenuating circumstances should be on a separate form.
